Study Overview

This clinical trial focuses on testing the efficacy of different digital interventions to promote re-engagement in cancer-related long-term follow-up care for adolescent and young adult (AYA) survivors of childhood cancer.

Description

This study will assess the efficacy of receiving emergency care at home versus in the brick-and-mortar emergency department.

Participation Criteria

Researchers look for people who fit a certain description, called eligibility criteria. Some examples of these criteria are a person's general health condition or prior treatments.

Ages Eligible for Study:18 Years
Sexes Eligible for Study:ALL
Accepts Healthy Volunteers:No
Standard Ages:ADULT, OLDER_ADULT
Inclusion CriteriaExclusion Criteria
  1. * Age \>= 18 years
  2. * Resides within Home Hospital geographic area
  3. * Lives in permanent housing (i.e., not in temporary housing such as a shelter)
  4. * Patient of a Mass General Brigham primary care provider
  5. * Primary care provider attests that their triage recommendation is the emergency department
  6. * Emergency care at home nurse triages the participant to the emergency department or urgent care
  7. * Patient attests that they intend to go to the emergency department
  1. * Insurance: workers compensation and motor vehicle accident
  2. * Lives in a healthcare facility (Skilled Nursing, Rehab, long term acute care)
  3. * Patient/caregiver cannot answer phone or door
  4. * Active substance use
  5. * Acute psychiatric concerns (e.g., suicidal ideation, even if passive)
  6. * Home safety concerns (e.g., intimate partner violence)
  7. * High-risk features:
  8. * Requires inpatient-level care
  9. * Requires specialty consultation
  10. * Requires physical, occupational, or speech therapy
  11. * Requires blood transfusion
  12. * Requires internal physical exam maneuver (e.g. rectal exam, genitourinary exam)
  13. * Requires imaging that is not available at home
  14. * Requires monitoring that is not available at home
  15. * Troubleshooting wound vacs
