Study Overview

This clinical trial focuses on testing the efficacy of different digital interventions to promote re-engagement in cancer-related long-term follow-up care for adolescent and young adult (AYA) survivors of childhood cancer.

Description

This study aims to compare a multidisciplinary clinical hypertension and social needs intervention to enhanced standard of care for hypertension management in primary care clinics with regards to hypertension control outcomes.

Participation Criteria

Researchers look for people who fit a certain description, called eligibility criteria. Some examples of these criteria are a person's general health condition or prior treatments.

Ages Eligible for Study:18 Years
Sexes Eligible for Study:ALL
Accepts Healthy Volunteers:No
Standard Ages:ADULT, OLDER_ADULT
Inclusion CriteriaExclusion Criteria
  1. * Adults 18 years or older
  2. * African American and/or Latinx
  3. * Uncontrolled stage 2 hypertension, BP \>/= 140/90
  4. * Patient is following with Rush primary care provider in eligible Rush primary care clinics
  5. * Patient has a smart phone (Android or iOS)
  6. * Patient has an email
  1. * Organ transplant recipient
  2. * On dialysis
  3. * Patient is already participating in another remote hypertension monitoring program
  4. * Patient is not interested in participating in the program
  5. * Patient has already participated in the E3 Hypertension program or the E3 Diabetes program
