Study Overview

This clinical trial focuses on testing the efficacy of different digital interventions to promote re-engagement in cancer-related long-term follow-up care for adolescent and young adult (AYA) survivors of childhood cancer.

Description

This project aims to adapt a computer-interface telephonic interactive voice response system that monitors symptoms and provides real-time, self-management coaching messages based on heart failure patient-reported outcomes.

Participation Criteria

Researchers look for people who fit a certain description, called eligibility criteria. Some examples of these criteria are a person's general health condition or prior treatments.

Sexes Eligible for Study:ALL
Accepts Healthy Volunteers:No
Standard Ages:ADULT, OLDER_ADULT
Inclusion CriteriaExclusion Criteria
  1. * Medical diagnosis of heart failure
  2. * New York Heart Association (NYHA) Classification of the Stages of Heart Failure Class I - IV
  3. * Ability to read, understand, and speak in English
  4. * Will be discharged home
  5. * Has daily access to any type of telephone
  1. * A score of 0 or 1-2 with an abnormally drawn clock on the Mini-Cog
  2. * Discharged home on hospice care
  3. * End-stage renal failure
  4. * Wait list for heart transplant
