Study Overview

This clinical trial focuses on testing the efficacy of different digital interventions to promote re-engagement in cancer-related long-term follow-up care for adolescent and young adult (AYA) survivors of childhood cancer.

Description

The purpose of this study is to evaluate whether an intervention called Pediatric Medication Therapy Management (pMTM) improves the identification and management of medication-related problems among children with medical complexity and polypharmacy.

Participation Criteria

Standard Ages:CHILD, ADULT
Inclusion CriteriaExclusion Criteria
  1. * 2-18 years old
  2. * ≥1 complex chronic condition (CCC)
  3. * ≥5 current medications (including prescription, as needed, and over-the-counter medications)
  4. * Receives primary care at Children's Hospital Colorado
  1. * Non-English speaking
  2. * Sibling or family member already enrolled in this study
