Community Health Equity Accelerator (CHEA) Pediatric Asthma Intervention

Study Overview

This clinical trial focuses on testing the efficacy of different digital interventions to promote re-engagement in cancer-related long-term follow-up care for adolescent and young adult (AYA) survivors of childhood cancer.

Description

The purpose of this pilot is to initiate an academic-community partnership and launch a multi-level intervention that includes an asthma exposure pathway (an online decision support and resource) and a health Promotora program that will provide culturally responsive asthma support alongside legal and medical services to improve access to asthma care and ultimately improve asthma-related outcomes in the New Haven region.

Participation Criteria

Researchers look for people who fit a certain description, called eligibility criteria. Some examples of these criteria are a person's general health condition or prior treatments.

Ages Eligible for Study:4 Years to 11 Years
Sexes Eligible for Study:ALL
Accepts Healthy Volunteers:No
Standard Ages:CHILD
Inclusion CriteriaExclusion Criteria
  1. * Oral steroid prescription in past 12m
  2. * ED visit for asthma in past 12m
  3. * Hospital admission for asthma in past 12m
  4. * Asthma control test (ACT) score at visit ≤19
  5. * Concern for asthmogenic exposure in the home (such as mold, pests, smoking, or pollution)
  6. * At least one missed follow-up appointment for asthma
  7. * At least one asthma exacerbation in the prior year
  8. * Interested in asthma education (such as education in how to prevent asthma or how to administer medications)
  9. * Concern for medication non-adherence
  1. * Living outside of New Haven Zip Code
